Cluster Lead - Jackie Reynolds

Jackie has been working within the Caerphilly locality since 1999, first as a Community Pharmacist in Blackwood then from 2004 as a Prescribing Advisor for ABUHB and as an Advanced Practice Prescribing Pharmacist for Oakfield Surgery Ystrad Mynach. With a MSc in Respiratory Medicine, her main clinical interest is respiratory disease, in particular sustainable respiratory prescribing and quality improvement work to benefit both patients and the planet.

Learning session on the appropriate use of YYF Minor Injuries Unit.

As part of the wider Advance Care Planning work, Glan yr Avon Residential Care & Nursing Home has just completed the Improving Quality Together (IQT) Silver Award. 

Caerphilly East NCN has awarded funding to Communities First to assist in the provision of X-POD, a 6 week health prevention and education program. The sessions are aimed at diagnosed pre-diabetic patients and those who want to reduce their risk of developing Type 2 Diabetes.

Communities First are also delivering Foodwise, an 8 week program developed to help individuals manage their weight the healthy way. The sessions, some of which are delivered at some GP practices, have been developed by Public Health Dieticians in Wales and linked with the Welsh Government Change 4 Life Campaign
